Description
Metformin can be used alone or in combination with other oral hypoglycemic agents or with insulin. However, it has some side effects like gastro-intestinal disturbances and low oral bioavailability which remains the main drawback of oral administration of metformin.
Cinnamaldehyde is an essential volatile oil found in cinnamon bark, has antihyperglycemic, antihyperlipidemic and antioxidant activities and it has a beneficial effect in treatment of type-2 DM. Cinnamaldehyde is subjected to first pass hepatic metabolism to produce cinnamic acid which has less pharmacological activity.
The aim of this study was to formulate Metformin and Cinnamaldehyde as rectal suppositories to get the advantage of this combination in treatment of DM and to overcome the gastric irritation of metformin as well as increasing the bioavailability of metformin and cinnamaldehyde.
The prepared suppositories were evaluated for their physical properties and drug release. Also, they were evaluated in vivo for drug absorption and hypoglycemic effect in rabbits.
Results showed that the selected formula (250 mg Metformin, 5mg Cinnamaldehyde and other excipients) in which polyethylene glycol was used as the base gave the best physical characteristics according to the USP and the total amount of Metformin and Cinnamaldehyde was released in 30 minutes.
Drug absorption from the selected formula was evaluated by measuring C max and T max in albino rabbits and they were 250 mcg/ml and 15 minutes respectively. Also, this formula was efficient in lowering blood glucose levels from 148 mg/dl to 85 mg/dl and to 63 mg/dl in 4 hr. and 6 hr. respectively.
Conclusion: Metformin HCL and Cinnamaldehyde combination was successfully formulated as suppository dosage form using Polyethylene glycol as a base and it gives very reliable in Vitro and in Vivo results and can be used as a promising alternative therapy in type 2 diabetes mellitus.
