Abstract

ABSTRACT
Background: Cluster of differentiation 40, a member of the tumour necrosis family of transmembrane glycoproteins. It was found that polymorphism of CD40 could induce CD40 expression, which increases pro-inflammatory cytokines, causing the development of autoimmune diseases. It has a significant impact on humoral and cellular immune responses in patients with chronic ITP disease treated with romiplostim. Aim: This study aimed to investigate the potential correlation between the CD40 single-nucleotide polymorphisms with different biomarkers in ITP patients on romiplostim therapy. Methods: A descriptive cross-sectional design was carried out from May 1st, 2025, to September 1st, 2025, in order to assess association of CD40 gene polymorphisms and cytokine profiles with romiplostim response in adult Iraqi patients with ITP disease in the Haematology and Bone Marrow Transplant Center, Medical City, Baghdad, Iraq. Result: The results showed the majority of patients were under the age of 60 (80.8%), more than half of them were female (66.7%), and, more than half of patients response to romiplostim effectively (66.6%). The result of the CD40 gene sequencing revealed the presence of the (rs4810485G>T and rs1883832C>T) among the studied patients. However, three other SNPs, namely rs1535045, (Position: 46118569), rs997412137 and rs774291557, were detected. Conclusions: The analysis of this study revealed all these SNPs showed no association with studied cytokine, including IL-10, IL35 and TGF-β, however, the result that IL-35 associated with increase response to romiplostim, while IL-10 associated with decrease response to romiplostim.
