Abstract
Background: Cisplatin-induced acute renal damage is a prominent dosage-limiting consequence of chemotherapy .Objective: The present study was designed to examine the nephroprotective effects of cilnidipine and lercanidipine in a cisplatin-induced acute kidney damage rat model. Methods: Thirty-six adult males Wistar rats were randomly allocated into six groups (n=6 each), including vehicle control, cisplatin, cilnidipine, lercanidipine, cilnidipine plus cisplatin, and lercanidipine plus cisplatin. Cilnidipine (2 mg/kg/day) and lercanidipine (3 mg/kg/day) were given orally for 7 days. Cisplatin (7.5 mg/kg) was given as a single intraperitoneal injection on day 5. Renal function, biomarkers of tubular injury, oxidative stress, antioxidant defenses, inflammatory mediators, Nrf2 mRNA expression and renal histology were evaluated. Results: Cisplatin raised serum creatinine, BUN, renal KIM-1, serum NGAL, MDA, IL-6, TNF-α, and IL-1β and decreased SOD, GSH, and Nrf2 expression. Both calcium channel blockers markedly improved the renal histological architecture and greatly ameliorated these cisplatin-induced alterations. The two medications had statistically equal effects for most of the measures studied; however, cilnidipine considerably lowered TNF-α more. Conclusions: Cilnidipine and lercanidipine exerted similar nephroprotective effects against cisplatin-induced acute kidney injury by modulating oxidative stress and inflammation and maintaining tubular and renal architecture. Cilnidipine had a marker specific anti-inflammatory advantage with more reduction in TNF-α whereas no overall benefit to lercanidipine was seen.
