Abstract

Abstract:
Abstract Background: Breast cancer is one of the most prevalent causes of cancer-related deaths in women. Trastuzumab, the monoclonal antibody used in the treatment of her2 positive breast cancer, has multiple side effects, such as back pain. This work examines the effects of genetic variation in the FCGR3A gene and the HER2 gene on the incidence of back pain during trastuzumab administration in Iraqi women. Methods: A cross-sectional observational study was conducted involving 50 Iraqi women diagnosed with HER2-positive breast cancer receiving trastuzumab. Ethical approval was obtained. Blood samples were collected for DNA extraction and genetic analysis. The severity of back pain was categorized as mild, moderate, or severe. Statistical analyses, including chi-square tests and ordinal logistic regression, were employed to assess associations between genetic variations and pain severity. Results: The study found no statistically significant association between the genetic variations rs396991 and rs1136201 and the severity of back pain (p = 0.584 and p = 0.397, respectively). Correlation analyses revealed weak relationships between treatment and disease duration, and genetic polymorphisms with pain outcomes. The ordinal logistic regression analysis indicated that demographic and genetic variables explained only a small portion of the variance in back pain levels (pseudo-R-squared = 0.044). Conclusion: This study examines the prevalence of back pain among Iraqi women receiving trastuzumab treatment for HER2-positive breast cancer. Moderate and severe pain conditions are common in certain genotypes. The distribution of genotypes showed that patients with the GT genotype for rs396991 were more likely to report moderate pain, whereas patients with the GG genotype were more likely to report severe pain. Similarly, a higher percentage of severe pain was linked to the AA genotype for rs1136201. Additionally, correlation analyses revealed weak associations between back pain outcomes and genetic polymorphisms.
